Sheffield City Council provide the operational management of the city-wide Move Well SchemeThe Move Well scheme is designed to help people with health conditions become more active. You would be part of a team supporting Service Users to be more active via a triage appointment which assesses their motivation, preferences and risk levels before directing them to a physical activity option that best meets their needs. You’ll be provided with motivational interviewing training and gain experience supporting people with health conditions within a structured intervention.You do not need previous experience delivering physical activity interventions, but it is essential that you enjoy speaking to people from a range of backgrounds and are comfortable having telephone conversations.We are looking for someone who can:Work as part of a team to support service users and physical activity providers.Deliver telephone triage appointments using person-centred approach that helps people focus on what matters to them.Lead by example by demonstrating excellent customer service, delivery skills, organisation and professionalism.
