Sheffield City Council is the operational lead for the scheme, which aims to:Connect people to support that meets their individual needs, enabling them to be active in ways that work for them.Provide healthcare professionals with tools to refer or signpost people to physical activity support as part of routine practice.Offer individuals access to a wide range of physical activity opportunities delivered by diverse providers in various locations and formats.Our vision is to enable sustained behaviour change by ensuring people with health conditions can access and maintain physical activity options in their community. Using a person-centred approach, we empower individuals to explore their motivations and find the support that is right for them.As Assistant Project Officer, you’ll take on a dynamic and varied role that combines operational leadership, team support, and community engagement.About the roleKey ResponsibilitiesPerson-Centred Support:- Deliver telephone triage appointments using a person-centred approach.- Use assessment tools to identify appropriate physical activity support.- Signpost and refer people to physical activity and community support.- Apply motivational interviewing techniques to help people overcome barriers.Service Quality and Development:- Collect and record outcome and monitoring data, contributing to service evaluation.- Complete administrative tasks to ensure smooth service operation.- Build knowledge and relationships with physical activity providers across Sheffield.- Respond to enquiries from health professionals and providers.Operational Leadership:- Coordinate day-to-day operations of Move Well.- Provide support and guidance to Prevention Workers.- Liaise with partners and stakeholders to maintain strong working relationships.- Assist the Project Officer with monitoring, reporting, and service improvements.Outreach and Awareness:- Represent Move Well at meetings, events, and conferences.- Deliver presentations and training sessions to raise awareness among professionals.- Promote the scheme through community engagement and partnership networks - Building relationships and raising the profile of Move Well regionally and nationally.
