Job Description
About the role:

Good health shouldn't be a privilege, and no one should be denied the chance to recover, rebuild or move forward because they can't access the care they need. At Harrow Road Assessment Hub in Westminster, you'll help break down the barriers that too often leave people experiencing homelessness without the healthcare, understanding and support they deserve.

Harrow Road provides short-stay accommodation and intensive support to people moving away from rough sleeping, many of whom have experienced significant trauma, poor physical and mental health, substance use and multiple disadvantage. As our Specialist Health Lead, you'll play a vital role in helping residents address unmet health needs, access treatment and take meaningful steps towards greater stability and wellbeing. Whether you're advocating for someone who has repeatedly been turned away from services, coordinating specialist referrals or supporting a resident to attend an important appointment, your work will help make healthcare more accessible and recovery feel possible.

You'll be a driving force for better health outcomes across the service, working alongside colleagues, healthcare professionals and community partners to ensure residents' needs are recognised and acted on. You'll provide specialist guidance to the team, strengthen relationships with local health providers and help shape more effective pathways into care. In a fast-moving environment where residents may be facing several challenges at once, your ability to build trust, challenge barriers and find creative solutions will make a real difference.

This is an opportunity to develop your specialist expertise, influence how health support is delivered and become a trusted voice within a passionate, multidisciplinary team. With opportunities for learning, collaboration and professional development, you'll help create lasting change for people whose health needs have too often gone unheard.

About you:

You are experienced in supporting people facing multiple disadvantage, including homelessness, mental and physical ill health, substance use and trauma, and understand how these experiences can create barriers to accessing healthcare.
You know how to build trust with people who may have had difficult experiences with services, using a compassionate, persistent and person-centred approach to encourage engagement and positive change.
You are confident navigating health and social care systems, coordinating referrals and advocating for individuals to access the treatment, specialist support and services they need.
You have experience working collaboratively with healthcare professionals and other agencies, and can use your knowledge to guide colleagues, challenge barriers and improve outcomes for clients.
You are resilient, resourceful and calm under pressure, with the tenacity to find solutions, manage competing priorities and respond effectively to health-related risks in a fast-paced environment.
About us:

We’re London’s leading homelessness charity – and we get things done.

In a city where hundreds are forced into homelessness every day, our work has never been more needed or more challenging. And we’re not shying away. We’re rolling up our sleeves to make change and helping over 10,000 Londoners every year. We prevent homelessness, provide safe places to live and give people the opportunity to rebuild their lives and transform their futures. And we never give up.

We’re here for Londoners wherever they are on their journey. We start with trust, building relationships that help people feel safe, supported, and ready to move forward. Every day, we put people first in everything we do, challenging injustice and barriers that keep people from the safety, stability and opportunity they deserve. We stand alongside people as they rebuild and shape a future that feels their own.

Joining Single Homeless Project means joining a team that’s bold, compassionate and determined to do better for the people we support and for each other. You’ll work alongside colleagues with lived experience, in a space that’s trans-inclusive, disability-friendly, and actively striving to be anti-oppressive and equitable.

We’re not perfect, but we’re real. We listen. We learn. And we push forward, together. Because this isn’t just a job. It’s a chance to lead with empathy, spark change, and help build a London where no one is left behind.
